Sign Up Forms.

The forms below can make your 'sign up' for the various sports a lot easier, as they can be filled in ahead of time.

The 'aaysa_sign-up_form.pdf' can be downloaded to your PC, opened using Adobe Acrobat reader and can be filled/typed in on the PC. This will allow you to save a - completed - copy to your hard drive and then print off copies to bring with you to the sign-ups.

TWO completed and signed copies are required for all sign-ups using this form.

The Parent's Agreement Form will have to be downloaded and printed for your signature. Please bring the signed copy with you to the 'sign-up'.
